Community Health Plan of Washington’s mission is to deliver accessible managed care services which meet the needs and improve the health of our communities and make managed care participation beneficial for community-responsive providers.

Total Rewards Statements

At the beginning of every year, CHPW calculates the total monetary value of the benefits you receive and creates a personalized statement for you. These Total Rewards Statements are mailed to your homes. 

Frequently Asked Questions

How do I access my statement?
Statements will be mailed to your home address. You can view your YTD compensation in UKG Pro by navigating to Myself > Pay > YTD Summary.

How often will I receive a statement?
Annually. After the end of the year, CHPW crunches the numbers and brings you the total value you have been paid. Statements will usually be sent out in the first quarter each year.
